  • Name of person, age, gender, address, village, District, State, community from which the person belong and which is recognized as backward class Under the Government of India, Ministry of Social Justice and Empowerment Resolution No. This is also certified that a person does not belong to the person/Section (Creamy layer) mentioned in column 3 of the schedule to the Government of India, Department of Personnel & Training O.M No. 36012/22/93 Estt.{SCT}, dated 08.09.1993.

Form SC/ST

Performa type of Credentials to be generated by the Nominee belonging to Planned Situation or Planned Group. The Competent Authority should be checked a man whose home is, Area title participate in Throw/ Group.
